Coaching Terms and Conditions - Hahn Health

Welcome to Hahn Health! These terms and conditions outline the agreement between you (the Client) and Vanessa Hahn (the Health Coach) for health coaching services. By using our services, you agree to these terms.

1. Coaching Relationship and Scope

  • Partnership: Health coaching is a partnership to help you achieve your health goals. It's a creative process to inspire you.
  • Independent Contractor: Vanessa Hahn is an independent contractor. This is not an employment relationship.
  • Disclaimer:
    • Health coaching is not a substitute for medical care. You must continue to see your primary care provider for medical advice and treatment.
    • When acting as a health coach, Vanessa Hahn does not:
      • Order labs.
      • Diagnose medical conditions.
      • Prescribe medications.
      • Interpret lab results.
    • If you are receiving services from Vanessa Hahn in her role as a primary care provider, a separate agreement applies to those services. In that role, she performs medical services including, but not limited to, ordering labs, providing referrals, prescribing medications, and diagnosing medical conditions.
    • In a medical emergency, contact your primary care provider or go to the emergency room.
  • New Hampshire Law: This coaching relationship is governed by the laws of the State of New Hampshire.

2. Services and Payment

  • Coaching Services: Coaching sessions will be conducted remotely via phone or video conference. The length of the program and the number of sessions will be as described in your coaching package.
  • Payment: Payment is due before the coaching program begins. If paying with a payment plan, late payments may incur a 5% late fee.
  • Support: Vanessa Hahn will respond to emails or Voxer messages within 48 hours during normal business hours (Monday-Friday, 8:00 AM to 5:00 PM).

3. Client Responsibilities

  • Engagement: Success depends on your active participation.
  • Personal Responsibility: You are responsible for your own decisions and actions based on the coaching.
  • Healthcare Consultation: You are encouraged to consult your licensed healthcare provider before making significant health changes.

4. Schedule and Fees

  • Program Duration: Coaching programs have a set duration. Unused sessions are not refundable or carried over.
  • Payment Terms: Payment is due upfront. Late payments will incur a 5% late fee.

5. Confidentiality

  • While not a legally protected relationship like a doctor-patient relationship, Vanessa Hahn will keep your information confidential, except when required by law or to prevent harm.
  • If you are receiving primary care services from Vanessa Hahn, that relationship and your information are protected, and Hahn Health follows HIPAA guidelines at all times to protect your confidential information.

6. Privacy and Data Protection

  • We comply with applicable privacy laws. Your data will be processed according to these laws.
  • In case of a data breach, you and relevant authorities will be notified as required by law.

7. Termination

  • You may terminate this agreement with 14 days' written notice. If you terminate the agreement, there will be no refund.
  • Vanessa Hahn may terminate this agreement at any time with written notice.

8. Cancellation Policy

  • Provide at least 48 hours' notice for cancellations. Missed sessions without notice may not be rescheduled.

9. Limitation of Liability and Indemnity

  • Vanessa Hahn makes no guarantees about the success of coaching.
  • Liability is limited to the amount you paid for services.
  • You agree to protect Vanessa Hahn from claims related to your use of coaching services.
  • Disputes will be resolved in New Hampshire courts.
  • Neither party is liable for events beyond their control (e.g., natural disasters).

10. Disclaimer of Warranties

  • Vanessa Hahn makes no warranties about the effectiveness of coaching. Past results do not guarantee future outcomes.

11. Entire Agreement

  • This agreement is the complete understanding between us. Any changes must be in writing and signed by both parties.

By using Hahn Health's services, you acknowledge that you have read, understood, and agree to these terms and conditions.
