Both patients and providers alike are fed up with the current system. This "traditional" model of healthcare places the primary focus on quantity rather than quality care. This is reimbursement/insurance driven primary care. A provider must document medical necessity in order to receive reimbursement for a specified amount of time spent or certain number of conditions addressed at each visit. Should this documentation lack support for said medical necessity, the provider may not get reimbursed at all. In this way providers often spend excess time documenting medical necessity rather than spending that time with the patient. This "traditional" model displaces the whole point of primary care... you, the patient.
Many insurance companies state that every beneficiary is entitled to and has direct access to a PCP, prompt referrals to specialists, home visits and "medically necessary" extra time with their provider. Now, ask yourself if you feel that you get all of these covered services with your current primary care office. Consider what that office would say if you requested a home visit next week.

I am sure you have heard the saying "if you want something you have never had you must do something you have never done." My name is Vanessa Hahn and I have been a Nurse Practitioner in Primary Care for the past eleven years. Along my primary care journey there have been many challenges to face.
Over the past 3-4 years some of these changes, both locally and globally, have affected us all.
With these challenges, I began to notice an evolution of focus in the primary care offices where I worked. There became a push to see more patients in less time. To encourage this push, there were incentive bonuses for seeing more and more patients more quickly. I recall receiving stern speeches about the drive to meet the numbers, and warnings that I was taking "too much" time with my patients.
Sadly, taking time with patient's no longer fits in the primary care system of today.
Then accessibility became a challenge.
I was forced to take on new patients despite the lack of availability for those patients already in my care. My patients would share how those extra moments typically spent in listening, teaching and including them in their own decisions made them feel truly cared for.
What my patients didn't see is that I was late for the next patients that were stacked into my schedule. They did not see that I was late every day to pick up my young children. They did not see that my extra time with them meant I had to login, finish notes, and answer messages that I had chosen not to write during their visits after putting my kids to bed each night. They did not see the strain this load was having on my personal wellbeing.
Every time I gave a young mother advise on the importance of taking time for herself or the impact high stress and lack of sleep and minimal exercise would have on her ability to care for those who depended on her long-term, the old adage of "do as I say, not as I do" would play in my mind. There had to be a better way...
I originally started my nursing career as an intern at LRGH in labor and delivery in 2008 and as an RN at the Ambulatory Surgical Center at the Laconia Clinic working pre-, intra- and post-operative nursing roles. I graduated Massachusetts College of Pharmacy and Health Sciences in 2013 and started working as a Nurse Practitioner in Plymouth New Hampshire.
After having my son, and wanting to be closer to home, I transferred to Lakes Region Family Practice in Laconia, until they closed their doors in June of 2023. It was at this time that I decided to embark on a journey seeking the possibility of something better. Better for my patients, better for my family and better for me. This is when I created Hahn Health.
I currently offer concierge in-home primary care services to those like-minded individuals who are also seeking a better alternative to the current broken healthcare system. My clients and I develop a unique relationship of mutual respect with a focus towards personalized convenience in accessing their insurance covered services. I prefer assisting patients in becoming active members of their own wellness and providing information and support to allow collaborative decision making along that journey.
My husband and I live locally and have two children that enjoy spending time in the outdoors of New England. Our adventures include white water rafting, canoeing, hiking, dirt biking, and simply mindfully enjoying our surroundings.

Hahn Health Members pay a monthly fee to be on a small panel which grants you and Vanessa the ability to develop a unique relationship where you do not feel rushed. Hahn Health also bills insurance for any covered service performed such as home visits or telehealth visits as is required by your insurance company.
Yes, you still need to keep insurance as Vanessa is credentialed with many insurance payers which grants her ability to provide care to clients in nursing facilities where concierge services are not traditionally done. In order to maintain credentials with these payers, Hahn Health must bill insurance for any covered service.
Hahn Health has a business office, but clients are seen in the comforts of their own homes. Vanessa performs house calls to clients in Gilford and the surrounding towns. This eliminates the stress of you needing to find a new office location, search for parking or organize transportation.
While Hahn Health is not affiliated with a specific hospital, Vanessa has Colleagues in many specialty offices who are affiliated with local hospitals. Any orders Vanessa writes, such as labs or imaging, are recognized by local facilities and can be done at these facilities as well as satellite laboratories such as Quest or LabCorp.
Vanessa is experienced in managing both chronic conditions and acute concerns including medication management as well as preventive health services.
Hahn Health is not a pain management clinic.
Yes. Vanessa understands the importance of a trusted medical team and collaborates often with various specialty groups in order to offer the most informed care.
Yes, Hahn Health is an established Professional Limited Liability Company (PLLC) and carries Malpractice Insurance as is required to practice in the State of New Hampshire.
There are not additional fees charged by Hahn Health specifically, but like any credentialed provider, copays and deductibles may be requested as they would for any medical visit based on your specific insurance plan. Any orders such as labs or imaging would be billed by the lab or imaging company and therefore not by Hahn Health.
The fee for Concierge In-Home Healthcare can range between $200-350/mo, which is paid quarterly and is not covered by insurance.
At this time, we service Gilford and the surrounding towns of the Lakes Region of New Hampshire.
